Operator Licenses

Steps for Completing Online Application: 
  1. Select: Online Application.
  2. Fill Out: Complete online application and submit.
    • If this is a new application, a copy  of your Alcohol Awareness course certificate (completed within the last 2 years) OR a copy of an operator's license that was issued to you in the last 2 years will need to be uploaded. 
  3. Pay: Payment is required at time of submission. Payment can be made via check (payable to Village of Windsor), cash , or credit/debit card (processing fees apply). Fee = $30
  4. Review/License: Staff will review the application and determine approval or send to the Village Board for consideration. Once approved, an operator license will be available online for printing. The operator license must be displayed or on person when serving.

 

Term Date: The license year runs from July 1 to June 30. Example: July 1, 2023 to June 30, 2024. Please note that if you are applying for a new operator license within the two month period before the license term begins, May to June, please contact the Village to discuss your option of a provisional license. Do not use the online form if you would like a provisional license.
